I've used the thumb and index finger most of my life but had to change to two finger picks over the last year due to my wrist condition. It's uncomfortable to me and in some cases throws my timing off so I'm slowly returning to the thumb and index finger again. As for speed, I've always used the thumb and two fingers for three notes on a vertical plain or four note crossover patterns. Speed has more to do with bar positioning and for some, the addition of pedals, so you can be equally fast with either pick choice. The decision rests on what gives you the best results for the style you want to play, and that doesn't take but a few weeks to weed out.
