When I was with the Everly Brothers I set the amp by what our sound men suggested for the venue. Most of the time the volume was around 9 o'clock with the master at around 2 o'clock. As for monitors, I would ask for enough steel to broaden the sound slightly but not override the amp. The only other instruments I had in my monitor were bass and keyboard. They were at the opposite end of the stage so I needed the bass for pulse and the keyboard for pitch.
